Carla has held significant leadership roles within complex healthcare systems, including Chief of Staff of a 1,200-member medical staff, Director of Critical Care, and a 30-year member of the Medical Executive Committee. She chaired and evolved a flagship Physician Wellbeing program over two decades, helping shift the culture toward more humane, sustainable models of care for the healthcare workforce. Her influence extends beyond her home institutions. Carla co-authored and helped nationally implement Language of Caring: Communication Essentials, a widely adopted curriculum and book strengthening connection and compassion in healthcare. She later developed Healthcare Now, a second curriculum focused on self-care, communication, resilience, and cultures of caring — teaching, facilitating, and embedding these principles in organizations across the country.
Carla has also served as an executive coach, leadership advisor, and international speaker, addressing audiences across healthcare, corporate, academic, and nonprofit sectors, with a consistent focus on conscious leadership, human potential, and the integration of performance with presence.

Carla is the founder of NightSkies™ and Boxcar™ International, initiatives devoted to creating spaces for reflection, renewal, and reimagination – particularly for those within medicine and other high-responsibility professions. Through her involvement with organizations such as 4Docs4Docs, MD Coaches, and Thunderbird Leadership Consulting, she has contributed to physician wellbeing, leadership development, and the advancement of more conscious and sustainable systems.
She also co-creates and leads immersive retreat experiences including Our Future Selves for physicians and Everything Under the Sun™ for women navigating complex and demanding lives – each designed as a sanctuary for reconnection, clarity, and transformation.
Carla is the creator of Bartering for Belonging (B4B) – a body of work that explores the often-unconscious ways individuals exchange authenticity for acceptance, and the resulting disconnection from self. Through writing, teaching, and experiential frameworks, B4B offers a pathway toward reclaiming wholeness, agency, and self-trust – both individually and collectively.
Emerging from this work is her vision for SAGE – a global, integrative platform designed to bring together diverse healing modalities, educational pathways, and communities of practice. Listen NowQ: You’ve had a 40-year career in medicine. What began to shift for you that made you realize there was more to healing than what you were trained to do?
A: After decades in medicine, I began noticing a quiet but persistent truth: I could treat disease, but I wasn’t always touching the deeper suffering. Patients would improve clinically, yet something unresolved lingered – fear, disconnection, meaninglessness. That realization wasn’t sudden; it accumulated. I started to sense that healing required more than protocols – it required presence, relationship, and an understanding of the human experience beyond physiology.
Q: Your journey into medicine is far from a typical one. Can you share how your early life shaped the way you practice and understand healing today?
A: “My early life taught me instability, uncertainty, and resilience before I ever learned anatomy. That contrast – between survival and later structure – shaped everything. I didn’t enter medicine from a place of privilege; I entered it searching for safety, identity, and worth. Today, I recognize how those early experiences allow me to sit with patients not just as a physician, but as someone who understands vulnerability from the inside out.”
Q: At what point did you begin to feel that the traditional medical model, while powerful, was incomplete?
A: “There came a point where I realized we were extraordinarily skilled at managing symptoms yet often disconnected from the root causes of suffering. Traditional medicine is powerful – but it’s not designed to address meaning, identity, or emotional truth. I began to feel that something essential was missing: the patient as a whole human being.”
Q: You’ve spoken about your transformational work through USM. How did that experience
change not just how you practice medicine, but how you see yourself?
A: “My work at the University of Santa Monica changed everything – not just how I practiced, but who I understood myself to be. It invited me to examine the unconscious patterns driving my life, including my need to prove, achieve, and belong. That shift – from external validation to internal awareness – transformed my relationship with patients. I stopped trying to fix and started learning how to truly see.”
Q: You often distinguish between ‘doing medicine’ and ‘being a healer.’ What does that
distinction mean to you?
A: “Doing medicine is about intervention – diagnosing, prescribing, managing. Being a healer is about presence. It’s about creating a space where patients feel seen, safe, and whole – even before anything is “fixed.” Healing isn’t always about curing; it’s about restoring connection – to self, to meaning, to life.”
Q: In your book Bartering for Belonging, you describe your life as a kind of laboratory. What were you discovering about yourself that ultimately became the foundation of that work?
A: “I remember that moment with a kind of searing clarity. I would say that my life became a laboratory when I began recognizing the subtle ways I had traded authenticity for acceptance. I was bartering – performing, achieving, conforming – in exchange for belonging. What I discovered was profound: belonging cannot be earned through self-abandonment. It must be rooted in self-truth. That realization became the foundation of the book.”
Q: “For someone who feels stuck, disconnected, or constantly striving to prove themselves, what does Bartering for Belonging offer them?”
A: “For anyone who feels stuck or constantly striving, Bartering for Belonging offers a mirror. It gently reveals the invisible contracts we make to feel worthy – and invites us to reconsider them. It’s not about fixing yourself; it’s about remembering who you were before you learned you had to earn your place.”
Q: You’ve observed that many patients are exploring healing approaches outside traditional
medicine, often without telling their doctors. What do you think is happening there?
A: “Patients are intuitively seeking wholeness. Many explore alternative approaches because they sense something is missing – but they don’t always feel safe discussing that with their physicians. There’s a gap of trust and language. What’s happening is not rebellion – it’s a quiet search for integration.”
Q: You’re now creating SAGE University, bringing together practitioners from across medical and healing traditions. What is the vision behind that, and why is it needed now?
A: “SAGE University is born from the recognition that healing is not owned by any one discipline. It’s a space where science, spirituality, psychology, and lived experience can coexist. The goal is to create practitioners who are not only technically excellent, but deeply self-aware. The world is ready for that level of integration.”
Q: If you look ahead, what does a more integrated, humane, and effective model of medicine actually look like for both patients and practitioners?
A: “The future of medicine is relational, integrative, and human-centered. It honors both data and intuition, both biology and biography. Practitioners will be trained not only in clinical skill, but in presence, listening, and self-awareness. Patients will no longer feel like cases – they will feel like participants in their own healing.”
Q: How has your understanding of belonging, both personally and professionally, shaped the way you think about health and healing?
A: “This is one those key, vital questions – I would say that the future of medicine is relational, integrative, and human-centered. The critical balance includes and honors both data and intuition, both biology and biography. Practitioners will be trained not only in clinical skill, but – to the extent it becomes reflexive – in real presence, deep listening, and generative self-awareness. Ideally, patients will no longer feel like cases – they will feel like “seen,” empowered collaborators in their own healing.”
Q: What would you say to physicians who feel something is missing in their work but don’t
yet have language for it?
A: “If something feels missing, have the courage to trust that instinct. You’re not broken – and neither is your training – but it may be incomplete. There is a much deeper dimension to this work that includes your own inner life. When you begin to explore that, your practice will transform – not because you’re doing more, but because you’re being more fully present.”

Over more than 40 years in medicine, Dr. Rotering has built a career marked by clinical leadership, service, and recognition within the healthcare community. She served as Chief of Staff of a 1,200-member medical staff at Banner Thunderbird Medical Center in Phoenix, and in 2019 was honored with a Lifetime Achievement Award as a "Healthcare Hero" by the Phoenix Business Journal, celebrating her decades of impact on patients and colleagues alike. She went on to found Boxcar International and its Night Skies initiative, extending her influence beyond the bedside into coaching, leadership development, and physician well-being. Her journey — from a childhood on the North Dakota prairie to decades in pulmonary and critical care medicine — has been shaped by a consistent thread of purpose, presence, and service to others.
